Why does a panic attack feel like dying?
Nothing has malfunctioned. The oldest alarm you own is sounding in a room with no predator in it, and most of what keeps it ringing is your fear of the alarm.
Somewhere behind you, in the undergrowth, something moves. You did not see it. You only heard it. And now your brain has to decide what that sound was, on almost no information, in less time than it takes to finish reading this sentence.
There are two ways to get it wrong.
You took it for a predator and ran. You were wrong. It cost you a few seconds, a hammering chest, and a story you have forgotten by evening.
You took it for the wind and stayed where you were. You were wrong. There is no evening.
Those two mistakes have the same shape and wildly different prices, and evolution is not a philosopher. It is an accountant. A nervous system that guessed predator too often lived to guess again. A nervous system that waited for proof left no descendants to inherit its patience. You are descended from the jumpy ones. That is not an insult. It is a lineage.
So let me say the thing that almost everything else follows from. Your nervous system was not built to make you comfortable. It was built to keep you alive. Those are different objectives, and they come apart completely in a quiet modern room where nothing at all is hunting you.
A panic attack is that system running, in full, exactly as designed, with no predator anywhere in the building.
The detector, not the fire
Deep in each side of the brain sits a small almond-shaped structure called the amygdala. The most useful way to think about it is as a smoke detector rather than a firefighter. It does not put anything out. It does not weigh context, or check the calendar, or ask whether you have an important morning tomorrow. It takes what the senses report and asks one question.
Could this kill me?
If the answer is anywhere near yes, it does not wait for a second opinion. It pulls the alarm. And this is the part worth holding on to for the rest of the lecture: almost everything you feel in the following minutes is not the danger. It is the alarm.
What the alarm actually does
The amygdala signals the hypothalamus. The hypothalamus turns on the sympathetic branch of the nervous system. The adrenal glands release adrenaline and noradrenaline, with cortisol arriving behind them. All of that happens in a fraction of a second, which is why you never catch it starting. You only ever arrive after it has started.
Then the body reorganises itself, all at once, around a single assumption: that you are about to fight something or run from it.
Not one of those is a malfunction. Muscles that might have to sprint need oxygen, so the heart delivers it, even though you are sitting on a couch and the most dangerous object in the room is your own phone. The pupils widen to let in more light and attention narrows onto whatever might be moving, which is why the edges of the world seem to go missing. Digestion is expensive and can wait, and that is where the nausea, the sudden dry mouth and the ache under the ribs come from. The tremor is not weakness. It is a muscle already loaded, waiting for a movement that never arrives.
The breathing deserves its own paragraph, because this is where the experience misleads people most. Breathing goes fast and shallow, and it feels precisely like not getting enough air. Almost everyone in a panic attack will tell you they cannot breathe.
In most cases the opposite is nearer the truth. You are moving plenty of air. What you are losing is carbon dioxide, faster than the body meant to lose it, and carbon dioxide is not simply waste: it is part of what holds the chemistry of the blood where the body wants it. Blow off too much of it and that chemistry shifts.
And there is the cruelty of the thing. Every one of those sensations is read by the detector as further evidence. The alarm produced them, and then hears them as proof that it was right to sound.
Why it feels like dying
People ask why it has to feel that enormous, and the answer is that the enormity is the point. Evolution has no interest in whether you feel like you are dying. It has an interest in whether you move.
A system that reported danger politely would simply be ignored. Picture the version that says:
Excuse me. At some point you may wish to consider moving.
You would finish your sentence. You would finish it and die in the middle of it. So the alarm is built to overwhelm: to become the largest thing in your mind, to feel like the most urgent fact in the universe, to be impossible to reason with while it is running. It is not exaggerating. It is doing the only job it has, at the only volume it knows.
The circle it makes on its own
Now put all that machinery in a body with nothing to run from, and watch what it does with itself. Say your heart skips a beat. Hearts do. You happen to notice it.
and round again, louder each time
That is a positive feedback loop, in the engineering sense rather than the cheerful one. Notice that no single step in it is irrational. Each one is a reasonable inference from the step before it. The problem is not any of the steps. The problem is the shape: a circle where the output is fed straight back in as input, and a circle like that has no natural place to stop.
Which is also how the fear changes address. It starts as fear of something out there. After two or three of these it becomes fear of the sensations themselves: of a fast pulse, of a warm crowded room, of the aisle in the shop where it happened once. The body has learned that its own signals are the emergency.
By now the thing you are afraid of is not in the room. It is your own heartbeat.
Why evolution never fixed this
The obvious question follows. If the alarm is this badly calibrated, why has selection not quietly corrected it by now?
Because it is not badly calibrated. Go back to the smoke detector. Burnt toast sets it off, and the whole house stands underneath it waving a towel and calling it stupid. It is not stupid. It is deliberately tuned to prefer a false alarm to a missed fire, and it earns back every false alarm it ever gave you on the one night it is right.
Engineers call this choosing sensitivity over specificity: catching everything, at the cost of catching a great deal that was never there. Evolution made the same trade for the same reason, and the arithmetic behind it is brutally simple. A lifetime of false alarms costs you a lifetime of bad afternoons. One missed alarm costs you every afternoon you were going to have.
It also explains why the detector gets touchier under certain conditions: too little sleep, a long stretch of stress, a great deal of caffeine, and above all previous panic attacks. A detector that has fired before is easier to fire again. The system was never designed to be fair to you. It was designed to remember.
The sigh you were already doing
Now the second half of the question, and I want to correct the name first, because it travels wrongly. It is the physiological sigh, not the psychological one. The slip is understandable, since the effect is felt psychologically, but the mechanism is plainly and unglamorously in the body.
You have done it many times today without noticing. So has your dog. A breath in, then a second smaller breath stacked on top of the first before it has gone anywhere, and then a long, slow letting go.
Why two intakes? The lungs are not two bags. They end in tiny air sacs called alveoli, and across the day some of them settle and partly collapse. The first breath fills what is already open. The second reaches what is not. Then the long exhale carries out the carbon dioxide that has been quietly gathering.
Nobody taught you this. It is built in, it runs on its own through the day, and other mammals do it too, which is usually a sign that a thing is old and load-bearing rather than clever.
Why a sigh reaches panic at all
During panic the breath is fast and shallow, and it stays that way because the alarm is holding it there. A sigh interrupts that pattern, and it does so from a direction the panic cannot easily argue with.
The lungs carry stretch receptors, sensors that report how inflated they are. They report to the brainstem, where the automatic side of breathing and heart rate is managed. A long exhale tends to shift the balance toward the parasympathetic side, the rest and digest side, largely by way of the vagus nerve, and the heart rate tends to come down with it.
What the body then has is evidence. Not reassurance, and not an argument. The detector has spent the last several minutes listening to your heart for its opinion on whether the emergency is real, and for the first time the answer coming back is slightly calmer than the one before it.
There is no count in this, and no dose. The shape is the whole of it: a breath, a second smaller breath on top of it, and an exhale longer than either. The timing belongs to your body rather than to me, and it has been doing this all day while you thought about other things. Nor does it delete a panic attack. Nothing deletes one. It lowers the volume slightly, and slightly is sometimes enough to keep the circle from closing.
It is not all in your head
One more thing, because people are told it constantly and it is simply wrong. "It is all in your head" is usually meant kindly and always lands as an accusation of having made it up.
The fear is generated in brain circuits. Nothing that follows it is. Adrenaline is a real molecule moving in real blood. The shift in blood chemistry is real. The heart rate is real and could be counted by a stranger. The muscle tension is real and will still ache tomorrow. A panic attack is produced by the brain and undergone by the entire body, and those two facts sit together without any tension at all.
What the answer is good for
So, to answer the question as it was asked. A panic attack is your survival system doing precisely what it evolved to do, at full strength, at the wrong moment, with no predator to spend itself on.
Knowing that is worth something, and it is worth being precise about what. It does not stop an attack, and I would not tell you that understanding a mechanism dissolves it. What it can change is the second fear, the one that grew on top of the first: the fear of the sensations. That one is learned, which is the good news, because learned things can be unlearned. Every time the heart races and the catastrophe does not arrive, the system takes note. It updates slowly, on evidence, the way it always has. It will not be talked out of anything, and it does not need to be.
You are not fragile. You are extremely well defended, by something very old, which has not yet been told where it is.
Nothing has broken. The alarm is simply older than the room you are standing in.
